2017-2018 Colorado State Directory Update Form
For COUNCILS, ASSEMBLIES and PERSONNEL
REPORTING TO THE STATE DEPUTY
All Councils, Assemblies and Personnel Reporting to the State Deputy are requested to
complete and submit a directory update form for the 2017-2018 State Council Directory,
after completion of council and assembly elections for the fraternal year 2017-2018.
The DEADLINE for submission of Update Forms for inclusion in the 2017-2018 State Council Directory is JULY 1, 2017.
Your assistance and cooperation in helping meet this goal will be greatly appreciated.
In addition, use these forms to submit updates that occur throughout the fraternal year.
Current DDs, GKs and FNs – Please provide directory information ASAP as part of completion of your current term of office.
